In order to ensure smooth processing into our Administrative HR System, please complete the following forms in ink and return to The Ohio State University at Mansfield, Office of Human Resources, 1760 University Drive, Mansfield, Ohio 44906.

You will only need to complete the State of Ohio and Federal (W-4) tax card if you have been employed with The Ohio State University during the past 12 months from the effective date of your offer letter and have previously completed all forms. The Social Security Form SSA-1945 is a new form that became required as of January 1, 2005, which you may not have previously completed.

The Mansfield Staff Advisory Council (MSAC) is an advisory forum for A&P and non-bargaining CCS staff at The Ohio State Mansfield. MSAC is your voice. We collect ideas from you and communicate them with Dean Gavazzi and other University leaders. * In accordance with federal law, The Ohio State University is required to verify the identity and work authorization of all new employees. As such, two (2) original forms of identification must be provided for verification purposes. If you are coming from out of the area or cannot stop by the HR Office before your start date, please complete and return all of the other forms and return them to the Office of Human Resources at your earliest convenience and make arrangements to stop by the HR Office upon your arrival on campus to present your two forms of I.D. and complete the I-9. Please refer to the last page of the “Employment Eligibility Verification” form for the list of acceptable documents. Due to federal guidelines, I.D.’s that are copied cannot be accepted.
