To ensure smooth onboarding into your new role, the Mansfield campus Human Resources team will partner with your supervisor to prepare your orientation to the university in advance of your start date. You will meet with Human Resources during your first day of employment, at which time all necessary documents will be given to you for completion. Below are additional resources for your convenience.
You will only need to complete the State of Ohio and Federal (W-4) tax card if you have been employed with The Ohio State University during the past 12 months from the effective date of your offer letter and have previously completed all forms. The Social Security Form SSA-1945 is a new form that became required as of January 1, 2005, which you may not have previously completed.
